Have you looked at rsynth? (poke around on sunsite) It's text-to-speech
output is not necessarily the greatest, but it is very compact and easy to
integrate.

Also, have you or anyone else looked at the Jabra headset? (www.jabra.com)
It looks like a comfortable in-the-ear headset / microphone combo that
would be very unobstrusive.
